Yes, there is a fundamental difference. Spammers post as much as they can, usually the most pointless stuff they can. Worse than that are those who create a bunch of almost empty threads to 'kill' all the other threads off the first page. Trolls are even worst : they create multiple accounts, often trying to make them look similar to an actual user's name, and sometimes copying the avatar and location to further trick people who don't look as closely as they should. These trolls are then used as spammer accounts, and it's extremely hard to find out who is responsible for troll accounts. However, you can tell a troll from the user they may be trying to copy - just check the member status and post count.

Spammers - users that post a great many pointless posts and/or threads in an attempt to disrupt the normal flow of a board.

Trolls - users that post intentionally inflammatory posts and/or threads in an attempt to rile up other users and sidetrack discussion.

Obviously, there can be a good deal of overlap.

Trolls aren't necessarily secondary accounts.
